Atlantic Health System joins the Cardiometabolic Center Alliance as a Charter Member, 1st in NJ

MORRISTOWN, NJ and KANSAS CITY, MO - The Cardiometabolic Center Alliance (CMCA), founded by Saint Luke’s Mid America Heart Institute, is honored to announce the addition of Atlantic Health System as a Charter member, and the first member in New Jersey.

As a coalition that seeks to establish Cardiometabolic Centers of Excellence nationwide aimed at transforming the treatment of type 2 diabetes (T2D), and related cardiovascular (CV) and renal comorbidities, the CMCA considers Atlantic Health System an excellent addition to the Alliance, with a recognized reputation for high quality patient outcomes, a valuable source of expertise aligned with the Alliance’s goals.

Together, our combined efforts aim to enhance the lives of people living with or at risk for developing cardiometabolic disease."

“Standardization of care for cardiometabolic conditions has long been a challenge in the primary care setting,” said Jeanine Bulan, MD, Associate Chief Medical Officer for Atlantic Medical Group, herself a primary care physician. “Coordinating and sharing information and best practices among the other members of the Alliance allows us all to level the playing field and enhance care for these patients. We are proud to be able to extend this coordination to patients in New Jersey.”

Work to improve the care of patients with cardiometabolic disease at Atlantic Health System has been system-wide effort under the direction of the Cardiometabolic Steering Committee, which is comprised of members from primary care, care coordination, medical specialties, bariatric surgery and diabetic education teams. 

The Cardiometabolic Center Alliance is a national collaborative of healthcare organizations committed to the mission of improving the quality of care and outcomes in patients with cardiometabolic disease. Across its aligned member sites, care is delivered via a standardized, patient-centered, team-based, comprehensive risk-reduction approach based on a novel care model designed and successfully implemented by Saint Luke’s Michael & Marlys Haverty Cardiometabolic Center of Excellence in Kansas City, Missouri.

Based on the initial data, under this model the rates of optimal guideline-directed medical therapy substantially increased over the national average, ensuring that patients with T2D receive care that is aimed at both optimizing diabetes management and providing comprehensive cardiovascular risk reduction with therapies proven to improve outcomes. These results are made possible by a unified team of clinicians from across the disease continuum, who focus on holistic, guideline-directed preventative care. Through their participation, Alliance members also contribute to a national registry that track treatment patterns and quality of care. As the Alliance enhances its capabilities, improves processes, and expands research opportunities on a national scale, the ultimate goal is improved patient outcomes.
